Can I start from a number other than 1?

Yes — set “Start numbering at” to any whole number.

Will blank lines get a number?

Not if “Skip blank lines” is on; blank lines are passed through unnumbered.

What is zero-padding for?

It keeps numbers aligned (01, 02 … 10) so lists line up neatly in monospaced contexts.
